Data Controller: The natural or legal person who determines the purposes and means of processing personal data and who is responsible for the establishment and management of the data recording system.
Data Processor: The natural or legal person who processes personal data on behalf of the data controller based on the authority given by the data controller.

PURPOSE AND LEGAL REASON FOR THE PROCESSING OF YOUR PERSONAL DATA
Your personal data is collected and preserved by the Company through different communication channels, in physical or electronic environments, based on several legal or commercial reasons, to provide benefits to you, to fulfil intracompany requirements/to continue or develop activities or to fulfil obligations arising from legislation.
STORAGE PERIODS OF YOUR PERSONAL DATA
Your personal data is stored by the Data Controller Company for the maximum period required for the purpose for which they are processed; the storage periods stipulated in the relevant legislation are considered in the storage periods of personal data. In fact, at the end of such period, your personal data is deleted, destroyed, or anonymized by the Data Controller Company.
Unless there is a special situation arising from the legislation, your personal data continues to be stored during the periods of legal statute of limitations regarding possible requests after the termination of your relationship with the Company, and at the end of such period, it is deleted, destroyed, or anonymized.

Request information about your personal data if it is processed,
Learn the purpose of processing personal data and whether they are used in accordance with such purpose, to know the third parties to which personal data is transferred at home or abroad,
Request correction of personal data if it is processed incompletely or incorrectly,
Request the deletion or destruction of personal data if the reasons requiring the processing of personal data cease to exist,
Request that the corrected or deleted data be notified to the third parties to which such data is transferred,
Object to unfavourable consequences if the processed data is analysed exclusively through automatic systems,
Request the compensation of the damage in case of damage due to unlawful processing of personal data.
You can send all your requests regarding the issue pursuant to the Communiqué on the Procedures and Principles of Application to the Data Controller published by the Personal Data Protection Board, in a certified mail or by means of a personal written application to our Clinic address given below, or to our Email address given below with secure electronic signature or Registered Email (KEP), mobile signature or via an Email you notified to us previously and available in our records. If the identity of the applicant is suspected in the application, we may contact you by phone or other means to confirm that the applicant is a beneficiary.
